90 Day Money Back Guarantee query

I have a question regarding the 90 Day Money Back Guarantee.
When I signed up with Plusnet, I believe I was entitled to a ‘90 Day Money Back Guarantee’.  I’ve just read the terms and conditions and I think I have to make a cancellation request in writing before the 90 days are up.  Could someone please let me know the exact date that my 90 days are up and the address I write to.

If you go to members centre then account details on the left it will tell you

The 90 day guarantee is effective if you migrated in to us. If you don't mind me asking, are you thinking of leaving and if so, why? Is there anything wrong with the service we can try and correct for you?

By ‘migrated’, you mean from another service provider as opposed to having broadband for the first time?
