My first question is:-
How many 4KHz tones are used by 21CN ADSL2+?
My router (A Plusnet Dlink) lists onlytones in the range 0-255 tones as used
I think that 512 tones should be used for ADSL2+...
Am I right?

Does your model of Router (and its Firmware release?) claim to support ADSL2+

I'm afraid I didn't understand much of the first link
However, I spotted that my router had a setting for the connection of "Best".
It said I wasn't supposed to change it - but I did anyway (to ADSL2/ADSL2+).
I've now got 512 tones - with the second 256 all zeroes
