A friend has had his exchange RFS date for 21CN set to the end of the month, does he have to do anything to get ADSL2+ , at the moment only gets 0.5MB should he see an improvement?

If it's the last date of the month it may be a place-holder and not a definite date.
When this says it's available your friend can either wait for the bulk upgrade or request an upgrade here.

Unless for some strange reason he's on a fixed 512 connection, he's not going to see much (if any) improvement.
