Flexor 500 help please

I've had a flexor 500 for a while now. Had a go at setting it up some months ago but even plusnet tech support could not get it going. I now really need this second phone  and I am aware that others have got it going. I plug it all in and it runs through Ethernet, LAN, Internet OK so all I need to do is configure it. I can get to the config account page.
Could anyone who has got this phone to work run through what they put in the account boxes? I can't even seem to find my SIP code / Username any more. I'm a total beginner at this and am struggling to get this going.  Many of the boxes do not correspond to anything in plusnet setup guide.
Help would be much appreciated.

Hi,
Here's a setup guide for the Flexor 500 although not for Plusnet. http://www.voiptalk.org/products/camrivox-flexor-500-voip-phone-setup.html
For Plusnet settings:-
On the first screen:
Name :- Plusnet
Sip Registrar:- sip.plus.net
Sip Port:- 5060
Sip Outbound Proxy:- natproxy.plus.net
Sip Outbound Proxy port :- 5082
The rest as per example
On the second screen:-
Service provider:- select Plusnet
Username:- your sip id
Authname:- your sip id
Password:-you sip password
Your name:- anything you like ( suggest your allocated number )
Everything else as example for now.
You got your sip id and password in an email when your account was activated.
