I'm guessing 22.214.171.124 is your router. It is clear from your tracert that is what is causing your problems before it even gets to the BT network and then to plusnet.
I suggest you reboot that device and re-run the WinMTR test and see if the packet loss goes away. If it does not it may be faulty and nees to be replaced. If you no longer get the packet loss on the router and you still have slow speeds then report the problem again.
