How do sites acquire the Spam URLs classification?

Access to Spam URLs is denied on all profiles which is fine, but …
Access to a research document (PDF) published by a company of which I am a client was denied and the checker says its URL is classified as Spam URLs. However that classification is not listed in the Add/Remove box so I can't suggest a change.
Please could Aladdin explain the logic behind this?

PS: I've downloaded the document using a connection that doesn't go through the eSafe box and can confirm the document contents are not spam.
