I have an expired rewards card, no excuse, i forgot about it. Has anyone any experience of replacing one?

Welcome to the foums.

Reading here - Solved: Expired reward card - Page 2 - BT Community - given that Plusnet is part of BT Consumer Division (along side BT Retail and EE) I would doubt that Plusnet's policy on unused reward cards will be any different - use it or lose it.

Though there have been some special exceptions - see https://community.plus.net/t5/My-Order/Pre-paid-reward-card/m-p/1766380#M16349

@wyjo - Hiya, sadly light bending powers are beyond me if it's expired as I have no way to get you another card now so much time has passed.
