Unfortunately, my Android device isn't "Rooted" so I can't provide any screen shots right now.
Install 3CX by downloading it from the Android Market.
Once installed and running, press the "Menu" softkey on your phone and select "Profiles"
Select "Add New" This takes you to the configuration screen.
Profile Name - Call it whatever you wish, in my case I have called it Plusnet
Credentials Screen
User - this will be your 7 digit extension number as found in your VoIP control panel

I don't know if people have experienced as much trouble as I have trying to get one of the recommended softphones to work with Plusnet VoIP. I was struggling for a whole day until I stumbled across this softphone called PhonerLite. It won't win any awards for artistic design but it has been easy to set up and use.

There are three (free) softphone's that I have tried for OS X; SJPhone, Xlite and Zoiper.
SJPhone is the one recommended on the PN help pages and if you want a quick set up and test it is adequate. However, it's one rather large failing is that it uses the default beep as a ring tone and there doesn't seem to be anyway to change this. Not good if you're away from your desk or have a quiet beep.
Xlite suffered from either a lag between answering the call and being able to speak or the answer button
